Eligibility to sit on the NCLEX-PN board

U.S.A. California

Hi all. I am an LVN student and will graduate by the end of this year. I have heard from friends who took the NCLEX board before that they extensively do a background check on the applicant before they let you take it. One of the things that I heard was your name on your SS card and driver license must match. Well, mine match but with an extra name on it.

When i moved to the USA, my legal name included my mom's maiden name. As an example if my name is Jane Sanchez (mom's maiden name) Smith. When i got married, i went to the SSA and changed my name to Jane Smith Robertson (I dropped the Anderson). No problems there. When i went to the DMV, i put the same name on the application but the DMV operator said that i have to keep the last name Sanchez. So on my driver license, my name reflects Jane Sanchez Smith Robertson.

Has anyone had this issue and did this pose a problem when sitting for your board?

Specializes in My first yr. as a LVN!.

I have a hyphenated last name as well and at times it has posed a problem. If you want to change your name you must go to the social security office and be issued a new SS card. The DMV has nothing to do with changing your name.. and they want what is EXACTLY on your SS card just like when you go to take your NCLEX it has to be EXACT on SS card as well as drivers license... so however you want your name you must first go to the social security office, then if you want your drivers license changed you must contact DMV... but also remember how you put your name on your nursing application is also what will reflect everything else ... good luck!
