What is standard practice for documenation of strips as far as what needs to be documented together and why. For example a patient has ecg, abg, PAP,CVP, etc. What 2 waveforms gets mounted together in the strips and why?

They should all be printed and mounted. Each line is there for a reason and should be documented as to what its waveform looks like. If you can only print two at a time, then bring the others up on a separate strip two at a time. It can be easily done. :)
does ecg HAVE to be with CVp or can CVP be run simultaneously with PAP?

I print these strips at the beginning of every shift & if there are any changes:
1) ECG + CVP
2) ECG + PAP
3) ECG + Aline waveform
4) ECG + PAWP (if I'm able to)
After printing them in the above combinations, I identify the ECG rhythm/rate & measurements in each strip, and then I simply label the other waveforms (& list the PAP & CVP measurements). Always make sure that you print each of the parameters with their scales, for without scales they don't mean anything.
The reason I print them with the ECG is so that you can id components of the waveforms.
An ABG is an arterial blood gas -- to obtain this you draw a blood sample out of the A-line & send it to lab. There is no waveform monitoring involved.
