sorry i can't share the exact questions or what audio but what basic assessment you have to do where sounds is heard?

i don't know about nclex 4000 but i used the nclex 3500 but did not finish the whole test qs bank. but what i can say is the lippincott alternate format questions for the nclex-rn was really helpful with this kind of questions.

nclex is supposed to be just about basic stuff so as long as you know what normal and abnormal sounds to what ever body system it is, you will be able to to pick the abnormal sounds.

good luck! you can do this.
