Documents and other resources

Hi,

I am currently a nursing student and have to complete a number of assignments. I was wondering if anyone knew of online locations that provided sample H&P documents. We were shown two examples in class for about 15 minutes and are now required to do a complete history and a mini physical (including appropriate documentation). I figure it may be helpful to look at a couple of other samples before writing my own.
