Do care plans get easier as you go

I am working on my care plan for case study in school. I am so confused I can't seem to make my brain think like a nurse. My pt has had a CVA c right side weakness. She is complete ADL 's and the only dx i can think of is skin integrity, impaired. my brain won't work.

I found this book from Mosby: 7th edition Nursing Diagnosis Handbook a guide to planning care. It goes by ADPIE (assessing, nsg diagnosis, planning, interventions and evaluations) it takes you step by step. I wish I had found this last semester when I had to write a careplan on every patient I had. I bought it when I had to do a case study this semester and could not for the life of me figure out 5 nursing diagnosis and interventions. I understand your frustration. Good luck!!
