They will most likely want to know your background, what areas you liked the most, your reasons for leaving the bedside, etc. You shouldn't have any real surprises there. If you have a background in cardiac care, spine care, or diabetes, let them know. That's where you'll see a lot of chronic disease patients who qualify for disease management, because they are the higher-cost conditions.
Focus on your ability to teach patients about their disease processes, how you handle conflict, and on how independent you are. And if you can let them know how interested you are in evidence based practice, that helps as well. If you're tech-savvy, they'll also appreciate that.
FWIW, I work from home, and I absolutely love it. I did not see a decrease in pay when I left the bedside, either. I educate patients over the phone, and even attend grand rounds & meetings over the phone, and in net-meetings. I loved working at the bedside, but I have it made where I work now. If you enjoy teaching, advocating for, and helping people keep their medical costs down, and empowering them to be involved in their health care, then DM is a great area to work in.

I have an upcoming interview over the phone. I was wondering if anyone could give me advice or what to expect. I have a 2 month old baby and this would be perfect to work from home. I could keep an eye on the sitter and know my baby is only a few feet away! I really want this job. I can't bear to work night shift anymore.
