Difficulty providing the VA with a required document

Hi there!! I was offered a position with the VA clinic in late July. I have been going through multiple hoops with Vet Pro, eQuip, and back ground checks. I even had my physical and finger prints done on Tuesday this week. I'm having an issue with Vet Pro. I had stated that I received on the job training for phlebotomy at a local hospital in 2000. I was asked to provide the certificate for Vet Pro. I no longer have the certificate and the hospital shredded all employee files older than 2002. I asked if staff in the Laboratory could sign a letter but no one remembers me. I wrote the Medical Director a letter and have not received any sort of response. I asked the lady in charge of my Vet Pro, if I could write a statement explaining the document no longer exists and have it notarized. Vet Pro lady has yet to respond. I feel really close to being cleared for everything except this one form. Does anyone have a similar situation? If so, please let me know how this issue was resolved. Thanks guys:)

Hi. Are you and LN or RN? I am an LVN in California working as a contract nurse for the VA. In Cali, our license specifically states whether we are IV/Blood Withdrawal certified. An RN is consdered to be proficient. I would just list your nursing license as proof. Don't let one little thing hold up the entire process, it takes long enough as it is.
