culture in Palliative care

Hi,

I have to write a paper on how culture is assessed in the palliative care setting. I have been trying to find admission forms which look at some aspect of culture (eg. religion diet, anything) with little luck. I anyone can help me out that would be great. I also was trying to find out how or even if a patient culture is assessed in palliative care. If so how, and to what benefit. Really struggling with this so if anyone has anything please help,
