Can someone give me examples of when to use a Discharge Assessment versus an OMRA end of therapy Discharge assessments? I thought if the patient was Medicare we are supposed to use the OMRA end of therapy Discharge assessment, but I heard that was incorrect.

you use the EOT if they remain on Med A skilled servics but are no longer havng therapy. Discharge assessments are for discharges from the facility, deaths, etc.
Would there be many instances when I would need to combine the EOT with the DC Summary?

March 1 was last day of all rehab therapies but resident will continue to require skilled service for managing a new onset unstable blood sugar elevation requiring daily blood sugar monitoring with a sliding scale insulin.
You choose March 4 as ARD for the OMRA EOT. On that same day, resident was found unresponsive and was discharged to hosp. Hence, combine OMRA EOT and Discharge w/ ARD March 4. The Rehab RUG rate will be paid until Mar. 1. The OMRA EOT RUG rate will be paid Mar. 2 - Mar. 3.
