if you suspect elder/child abuse what intervention should the nurse make?1. report to proper authority

my review center told me to document findings while in the saunders book it says to report findings...i dunno which one is right, I'm confused.
you are doing both, you do report because child could be in danger, pt safety, documentation is not an emergency, your pt may be in immediate danger

A nurse is taking care of a client on contact isolation. After nursing care has been performed and upon leaving the room, the nurse removes protective items in which order of priority?
a)Unties the gown at the waist
b)Removes the gloves
c)Performs handwashing
d)Removes the mask and eyewear (goggles)
e)Discards the gown in appropriate receptacle
f)Unties the gown at the neck, allows it to fall forward toward the shoulders, and removes it.
Which order would you put this in??
I thought the order of taking PPE was Gloves, Goggles/Face shield, Gown, Mask/respirator...but apparently according to the answer to this, it's not.

RATIONALE: At the door of the room and next to an appropriate receptacle, the nurse unties the gown at the waist but does not remove it yet. The nurse then removes the gloves. The gown is then untied at the neck, allowed to fall forward from the shoulders, and removed. The gown is then discarded in the appropriate receptacle. The nurse then removes the mask and goggles and discards them. The hands are then washed.

According to Kaplan:
When applying:
(1) Gown (2) Mask (3) Goggles (4) Gloves
When Removing, do it in reverse:
(2) Gloves (2) Goggles (3) Mask (1) Gown
I just think like:
When you're putting it on, start from bottom (body) to top (head) to extremities (hands).
When you're taking off the equipment, just do it in reverse.
first question is on the child abuse... if i were to choose with two choices above,, i'll chose the i'll report thing since it says there that you already suspect something and it is our role to report suspected child abuse ...document it without doing nothing is a bad move.. the later questions talk about what to remove in order... my technique is removing it in alphabetical order

then hand wash
when entering
handwash
mask
gown
goggles
gloves
just the reverse..
