What are the most common incompatiable drugs/fluids?

I usually have to look those up everytime. At the unit I had worked on, there's this chart that shows which medications are compatible with TPN. If they are compatible with TPN, then for sure, they are compatible with pretty much any fluid. Some of these drugs include flagyl and gentamicin.

I know Ampicillin is NOT compatible with TPN. I was told by one person that it is only compatible with normal saline but was told by another it is also compatible with D5. On the safe side though, I still dropped amp into normal saline.
