Do you customize your comfort kits for each patient, or do you have a standard
kit sent out for all patients?

We have the standard kit from Hospice Pharmacia.

standard--but if a patient is prone to seizures we are to check with the primary MD about deleting the Haloperidol. Meds are only inhome--NOT activaterd until time of need and then each dose order is customized to the patient.

We have comfort orders that we have the attending sign on admission ( no ppharmacia) and the admission nurse usually will have filled after assessment meds needed based on the diagnosis and condition. The case manager the will have filled other meds as needed. The orders were complied by our Medical Director, our pharmacist and we nurses.:typing

We are here in Kona, Hawaii on the Big Island, and have been affected by the economy too. We just started customizing our comfort kits. The 3 main meds are liquid morphine, 10 tablets of ativan 1mg and 2 scoplomine patches. If I (admission nurse), think the pt will need tylenol suppositories or dulcolax suppositories, then I add those. Does this help?

Our EKit contains Roxanol, Haldol, Ativan, Tylenol suppositories, biscodyl suppositories and Atropine gtts.

our kits are standard but adjusted based on allergies/hypersensitivities. Kit includes: Morphine sulfate, ativan, hadol, compazine tabs and supps, levsin, tylenol supps, and ABHR (ativan/benadryl/haldol/reglan) supps(for n/v).
We also use a seizure kit if needed which has ativan supp and dilantin supp.
We will subtract according to allergies/sensitivities, but that is our standard ekit.

Hospice Pharmacia and other pharmacies offer choices in the contents of the kits...
w/ or w/o levsin for instance, or scop TD.
Mostly it depends upon what your agency contracts as part of the "per diem" with the pharmacy provider.
