Hi there,
I recently discovered my co worker has been working with a lapsed LPN license the last 4 years. I will report it to the board anon but my question is will the practice face problems because of this? It's a small dr's office and I do not want anyone to loose their jobs because of one person's mistake.

I would make this fact known to the owner of the practice if it concerns you. He should be given the opportunity to rectify anything that might need rectifying. All it might take is to temporarily reassign her to desk duty until she squares herself away. The boss will thank you for the heads up.
Management has been tipped off and knows...I was told legally you have to report it once you know someone is working without a license, not true???
Legally somebody has to report it. For that matter, now that the doctor knows, it is just as much his reponsibility as your's. Do what you need to do and keep a copy of the letter.
