Does anyone have a worksheet used by Nurses and Nurse assistants to communicate information regarding patient care during the shift? Maybe something with VS, I & O, Ambulation, turns, etc.

We just put everything in the chart. No extra sheets. As far as turning etc, we just inform the nurse with "updates" throughout the shift.

We have a CENA ADL chart where the aids chart their AM care, PM care, showers, etc. They also chart I&O, BM's, and any other significant thing that may have happened during thier shift. It is very helpful for the nurses to go and look at the CENA charting when they have questions about I&O or anything of that sort.
