Hi guys I'm new to this site but I'm sort of in a pickle. My manager informed me that a family reported me for neglect on their father and that she had to report it to the state. I am to write a letter to her to submit. The issue is our facility is always understaffed and we are overworked with 11-13 patients average. It was a busy day and I was training a new aid. The patient I supposedly neglected spoke no English and there was not a translate around. I ask the patient and used hand signal if he need help washing up but he was confused and shooed me away. I was trained to never force a patient to wash. It's not like I was to start stripping him. Mind you he can ambulatory and go to the bathroom on his own. The family came later and was furous that he wasn't washed. Do you guys think I will lose my license? I love being an cna and would be heartbroken. I can't imagine myself not in the medical field and I'm currently applying to nursing school.

In MA CNAs do not have a license. There is a certification and I would think you'd be very unlikely to lose it.

Agree with the PPs.
In your letter, I wouldn't go into it being a busy shift - they all are and it's to be expected - give them specifics about the interaction: what time(s) you approached the patient, and describe how you did it and the cues/gestures/signals you used.
In very clear but neutral language describe how the patient reacted (similar to above: hand gestures they used, verbalizations/head shaking etc).
You can also reinforce the self-determination piece (i.e. the patient having the right to refuse that care), but also tell them what your plan would have been to follow up (trying again in x hours, passing on to next shift, tracking in a shift sheet/log etc - preventative things that show them proactively that the patient wouldn't have been neglected).
