CNA flow sheets

I'm new to my job in this facility. The CNA flow sheets look like they have been recopied 34,972 times! The language on the sheets match neither the MDS nor the MMQ language. I could spend my time formulating a new flow sheet using the MDS language with the definitions of limited vs extensive assist, or I could ask one of you what you use. My last facility used CareTracker which I loved but this company is not going to spend the money.

i'd be interestd in your CNA flowsheets. i'm new to my job and i'm trying to work with what we have but i'm always looking for something better....

I would also be interested in any forms to help adl scores. today a nurse brought me a flow sheet with 5 million holes and I know it is the form it is too busy for them also anyone have any good meal intake forms??Please send to [email protected] would so greatly appreciate it!!
