Does anybody know how to go about obtaining your CNA certificate number in California?

I thought the state would send me something in the mail after I passed my tests, but I've received nothing for 2 months now. I'm going to try sending in this form next and see what happens.

I think that form is for out-of-state applicants transferring their certificates to California.

I got my certificate last year, but I misplaced the certificate.

I will call the Department of Health directly tomorrow, and I will post their response.

Thanks, I've gone over their website multiple times since taking my tests and found nothing helpful other than that form. There's this FAQ, but it doesn't answer my question since I passed the tests and the background check and have heard nothing other than the letter stating that my passing scores were being forwarded to the state. I thought perhaps there was a backlog of certifications due to the Governor laying off the part time and temporary employees during the budget stalemate.

Thanks for any info you may determine. I wish it was as easy as the RN database where you just enter your last name and the certification pops up, but the CNA database requires that you know the certification number in order to search for anyone. I didn't expect it to take this long to be able to start working.

In my state the license numbers are posted on the Department of Health and Human Services website. My license number just posted this weekend (I passed my state exams on 10/10). I don't know if I'm going to get anything in the mail. I wouldn't be surprised if I don't.

In California, you must know the certification number in order to search the database for CNAs. http://www.applications.dhs.ca.gov/cvl/

For RNs you don't need the certification number...it's kinda weird, but 2 different state departments handle the certification data so this happens.
