charting

I'm not happy with my charting-on the run sheets.I . Any ideas ,suggestions out there there's not much room but I really feel like I need to do a better job-just not sure how .I know there's not much room on the sheets to write so I want what I do write to be significant and useful - any ideas or suggestions

When I needed more room to write up a incident about the patient run, I would attach a blank physician progress note to my patient's flowsheet. You would need to indicate on the flowsheet, such as page 1 of 2 or see page 2, so everyone would know there is more info available.

Not quite sure if this is what you was addressing. What kind of info did you want to chart about?

sleeping or awake, alert, c/o pain, access visible, BFR/UF being tolerated, pt sitting upright, or reclined, resolution of any earlier c/o headache, cramps, etc., any interventions such as med administration, BFR or UF alterations, any teaching done (pt eating fritos, counseled pt re: binders)

Is that what you're looking for? I think noting a visible access q 30 minutes or so is a very good thing to do to CYA in case something were to happen later. (low hgb, were the needles leaking during last tx?? Nope, you documented every half hour!)
