Hi everyone! Let me introduce myself quickly: I'm Guillaume, 25,French RN wishing to work in the USA. I'm applying CGFNS CES, and I have an issue with the Request for Academic Records/transcript form. I do not understand what they mean by "Patient Care Environment" and "Simulation", both are requiring hours attained in "Acute/Community/Long-term Care". I'm kinda lost and I can't send the files without filling that part.. If someone, any US RN or international nurse that already filled that part could explain me what does it include/mean?
Thank you for your help!
Ps: I join a picture of the part on the form

I believe that form should be filled up by your university registrar and also, they should be the one to send it to CGFNS.
Thank you for your answer! I do know that my school have to fill the form, and they do. But they need my help to translate the form and what they mean in the subject areas (it's the first time they do it and they do not have a good english level).
By the way, don't you know what it refers to?

Simulation = non-clinical areas or lab either on mannequins or other students. Where you practice skills and demonstrate competency before working with actual patients.
patient care= supervised nursing skills in the care areas listed either under a nursing school clinical instructor or licensed nurse as preceptor.
One must have classroom/theory and clinical/practical hours in all domains adult, geriatric, pediatrics, obstetric/maternity, and psych/mental health plus community health Theory/classroom instruction in science, anatomy & physiology (with lab), microbiology, nutrition, pharmacology, sometimes chemistry, English, communications, developmental psychology and sociology.
Acute- inpatient acute care facility usually OR, L&D, critical care, medical-surgical, pediatrics and inpatient mental health
long term care is nursing home, subacute rehab long term facility based care
community could be community outpatient or partial mental health, community clinic, home health/visiting nurses"
Wow! Thank you for your your time and your answer! That's complete and I think I got it all! Thank you very very much, that's helping me out!

JustBeachyNurse => Have you ever fill that form?
No. I'm US educated, I'm just familiar with the terminology as it's pretty standard in US schools & BoN regulations

Hello GuiguiFr,
I am having same issues as to you did. CGFNS wants me to re-do the Academic Records form, and University does not have any clue how to fill this out since the forms are kind of new and not what they are used to. Can you kindly let me know how you filled the new Academic Records form? I want to make sure I do it right after all the run around I got from CGFNS.

Hi , can anyone please explain some things to me as well ? My school is struggling with this paper and they told me to contact cgfns and figure out how to fill the form . I called , the lady was nice to me , she was trying to help but I felt like she did not have all the info . Now at least I know that gerontology is not necesary but there are other subjects that I did not study . Nutrition in my country is a whole different degree we did not have it in our curriculum . Maybe if I could understand exactly what do you study in those I could write them down .
Community health concepts ??? Is this public nursing and social assisstance ??
Physical assessment = is this practical skills ??
Ethical consideration = is this ethics and deontology ??
Healing Counseling / human growth and development throught the lifespan ??
Interpersonal relationships ??
Leadership in nursing ??
Legal aspects in nursing= legislation in nursing ??
Personal and family health concepts ??
Professional roles and functions ??
Unfurtunetly these are terms that we don't use and don't understand , but if I could understand what was studied in these I could tell the school and they could fill out my papers .
