CES report on Illinois BON

i've posted this in one of the threads here about the application process in illinois bon. anyways, this thread would be least likely to be helpful for ces application. again sorry for a lot of questions and hope you can help me with this matter. much thanks everyone! god bless!

just filling up my ces application form. hope you could help me in some of my questions:

  • authorization to release information form - what should i write below the authorized agent? such as follows:
    • contact name
    • organization name
    • address
    • telephone as such.

    [*]what is the best mode of payment? bankdraft or credit card (through the credit card form included in the ces packet)

    [*]application form:

    • question #9 : where will i get my citizenship id number?
    • question #12 :
      • i understand that for the pre-health profession education, it's from your elementary up to highschool. question is: do i need to pass both of my elementary and highschool diplomas that i have here presently or do i still need to request from my alma mater those diplomas? (all are written in "tagalog")
      • for the healthcare education, since i studied in 2 nursing schools (just to finish my course) do i still need to indicate the 1st school i entered my nursing? (though i did not finish my nursing course there since i transfered to another school)
      • are there any instances that they will question your 1st school since you did not finish your course there and not earning a diploma?
      • correct me, i only need to pass the one form of the request for academic record for ces to my 2nd school where i finished my nursing degree right?

      [*]question #14 : correct me, that the report recipient is no other than [color=#292526]continental testing services inc. p.o. box 100 lagrange, illinois 60525-0100 right?

      [*]question #15 :

      • we only need the healthcare profession and science report right?
      • do i still need to have a cgfns language report: english? i want to take ielts instead of toefl ibt, will this help in order toefl-ibt be waived?
      • is there any other requirments needed to be requested at cgfns?

The CES report is completed by a separate agency, CGFNS, and there is not a separate report that is generated for Illinois specifically. You are permitted two entities that they will forward the report to that is included in the fee that you pay, any additional ones and you will pay extra for it.

If you wanting to work in Illinois, then you list that state there as one of them.

You do not have an agency that is submitting your application for you and that is what the authorized agent is, you are paying your own fees for this, so there is no one else to send it to, or give permission to for them to access your file.

Any school that you attended for nursing, or anything else, must be listed. It says to include all, not just where you graduated from. Please be aware that whatever is included on this form will have to be the same that is included on everything else, including anything for immigration, and they must all match.

If you have read any of the other threads here on this forum, we always recommend that you go for the full course report. If you decide to go on with your schooling later on, then you will need the full report, no sense in paying twice for something that could be done now.

There actually is much on completing the CES application on this forum, you just need to do a search for it. And we do ask that you post the same thing in only one thread and wait for your answers there, makes it easier for everyone else here to follow.

If your training was 100% in English, then Illinois will waive the English requirement for you. But the English is still required for immigration and you can use the IELTS for that.
