You are required to respond to the following case study:
Mr. JACK is a 29 year old man who lives on a property 150 kms from the nearest health care facility. Mr JACK tore his left lower leg open on a piece of dirty barbed wire seven days ago and has been admitted to the St. Elsewhere Hospital with an infected wound. The pathology results show the causative organism of the infection to the left lower leg is Methicillin-resistant Staphylococcus aureus (MRSA). Mr. JACK was admitted to the ward at 0300 hrs and his observations are as follows.
Time 0300 0400 0500 0600
BP 110/65 118/68 134/73 141/81
Pulse 86 90 94 101
RR 20 21 22 25
Temp 36.5° C 36.4° C 36.2° C 36.1° C
Sp0299% RA 99% RA 99% RA 98% RA
Pain Score 5/10 2/10 4/10 5/10
Mr. JACK was administered 2.5 mg subcutaneous Morphine for pain on admission to the ward and the Medical Officer has prescribed that this can be repeated at four hourly intervals if required. Mr. JACK has also been prescribed appropriate intravenous antibiotics as charted on the medication sheet. He is on bed rest with toilet privileges and can mobilise for a shower.
On your arrival to the ward, Mr. JACK is in work clothes and is reporting 5/10 pain in his leg.
TASK
Prepare a nursing care plan using the template provided on Moodle for the hours of 0700 to 1530 that clearly identifies your planning, organisation, documentation and participation in the provision of evidence-based care for Mr. JACK. Use the Clinical Reasoning Cycle as a framework for discussing the processes you use in constructing this care plan. Outline the rationale for three nursing diagnoses and interventions implemented in the care of Aaron showing links to the ANMC competency standards. You should utilise relevant peer reviewed journal articles, text books and web sites. You do not need to include discussion of the prescribed medications. Consider the nature of the infection and possible consequences of mismanagement.

Welcome!
We are happy to help but we need to know what you think first!
Well thanks my learned friends. I have already developed the care plans which consist of the nursing diagnosis, outcome, intervention and the evaluation but my problem is the rationale and how to link it up with the australian nursing and midwifery council competency standards and also how to fuse the clinical reasoning cycle into the essay. The teacher said we need to look into the psychosocial issue of the patient which i can't seem to understand.
So....I looked up the practice standards.http://www.nursingmidwiferyboard.gov.au/Codes-Guidelines-Statements/Codes-Guidelines.aspx
How do the interventions you chose for your patient fit within the standards of care set forth by your governing board. How does evidence based practice show how the intervention you chose for your patient is within the standard for that diagnosis/intervention. I need examples to better explain to you.
As far as the psychological aspect...how will a tough old independent farmer living in the outback deal with hospitalization and restrictions for healing. Do you feeelthat he is going to be patient with the restrictions or fight them tooth and nail to get back to his farm? What would happen if this farmer doesn't go as told and develops gangrene or tetnas? How would this affect his life?
