Careplan Assessment Help

I'm sure everyone is either at work or sleep and not thinking about allnurses.

But for those of you that are up can you help me?

I'm soooo sleepy. But I need to finish this dumb careplan.

I'm supposed to rank my nursing diagnosis from most urgent or life threatening to the least threatening.

This is what I picked...tell me what you think....

  • Diarrhea
  • Impaired physical mobility
  • Pain
  • Anxiety

I never rank them right because I usually don't think of too many diagnoses as being life threatening. YAWN... sooo sleepy. 29_3_14.gif

nursing student here who also gets it out of order sometimes, but going to give it a try! :)

1. Pain

(we have always been told pain is a priority)

2. Impaired physical mobility

3. Diarrhea

(tough choice to me as to #2 and #3, but I am thinking that impaired physical mobility can lead to pressure ulcers, while diarrhea can be medicated ?)

4. Anxiety

I would rate them as such 1) pain 2) diarrhea 3) impaired mobility 4)anxiety

Pain first because we have always been taught that a person in pain is very rarely interested in what you think he/she needs to be doing for them.

diarrhea because of all the problems it can cause with electolyte imbalances, and dehydrating a patient.

impaired mobility can be helped by lowering a persons pain so they can ambulate more.

anxiety just because its the last one. But all the above can contribute to a persons anxiety level. Taking care of them will reduce anxiety as with a little teaching and TLC

Thanks guys, I went to sleep before I got your replies. I think I listed them as

Diarrhea

Pain

Impaired Physical Mobility

Anxiety

It looks like Pain is the choice for number one. I didn't think of it that way because my patient's diarrhea was related to infection by C. diff in her stool. The pain was due to a total knee replacement. In my mind I felt that the infection needed to be dealt with first before the pain.

I guess it should vary for each patient? Like, someones pain that is due to an MI as opposed to someones pain from a fall.

What about the ranking scale? If one patient says his pain is a 2 and the other a 8 then would my ranking be different in their careplans?
