Patient admitted to facility post Closed Head Injury. After his arrival it was determined that he was much more lethargic than admission info indicated. He was transferred back to Acute Care within 48 hours. Nearly all of the CAA's were triggered, but he wasn't in the facility long enough to implement all of the care plans. Is it okay to answer "NO" to the Care Plans that hadn't yet been implemented? They were addressed as nursing assessment notes, but no care plans.

Since stay is less than 14 days, you don't even need to complete an Adm MDS. An entry record and a discharge is all that's required. Just change the current assm't type to a discharge (combined w/ a 5-day if applicable) and forget the CAAs.

You are working yourself to death. Follow what Talino said. If the resident was medicare, you may need to complete a PPS assessment as well but you don't need an admission assessment
