Care Plan

Hello everyone!

Well the semester just started and we haven't even been to class and we have a "study guide" due in my Adult Heath class. I don't even know how to start on a basic care plan, we haven't been to class yet, and we have a list of them to do. Can someone point me in the right direction?

Here is a small portion of what is being ask....

Develop a care plan to manage the following symptoms in the dying patient

  • fatigue management
  • pain management
  • dyspnea management

ok that is a sample there are many more, so if anyone can give me just a glimpse of what it is they are asking me to do I would be deeply in debt.

care plans are written documentation of the problem solving process.

  1. assess
  2. determine the problem
  3. plan the solution
  4. implement the plan
  5. evaluate the outcome

fatigue, pain and dyspnea are patient problems. there are nursing diagnoses for fatigue ([color=#3366ff]fatigue) and pain ([color=#3366ff]acute pain and [color=#3366ff]chronic pain). that means there are symptoms that can be listed out for these two conditions. if there are symptoms then there are treatments that can be aimed at them. that is what the nursing interventions in a care plan are about. dyspnea is the cause of all kinds of other problems as well ([color=#3366ff]ineffective airway clearance, [color=#3366ff]ineffective breathing pattern, [color=#3366ff]decreased cardiac output, [color=#3366ff]impaired gas exchange, and impaired physical mobility). and those can be care planned for as well. what you will do is focus your care plan to the specific needs of a patient who is probably not likely to be cured of their disease.
