Sorry. My dx was
impaired skin integrity related to swelling and inflammation as evidenced by break in skin and decreased mobility.
I meant swelling not sweeping. So this is not acceptable? I will invest in that book, however I have to turn in the care plan in a couple of days. Thanks for letting me know about the book! :)

We won't know how it is suppose to be worded truly until we have a history and assessment data on this patient. So essentially, what I'm asking is, what's going on with your patient? Paint a picture for us.
Also consider this:
Cellulitis is an infection of the dermis and hypodermis. The inflammation/rubor is due to the infection - well and in reality due to your body fighting it (vasodilation causes the inflammation - as it attempts to bring resources to the area of "incident".)

Okay. It looks like you may have things flip flopped. Think about your own wording for a second. However, first consider the following.
Example:
Excess fluid volume r/t excess fluid intake as evidenced by jugular vein distention and crackles upon auscultation.
Translation:
Excess fluid ((because*R/T*)) the patient drank so much water ((I can tell you*AEB*)) they have excess fluids because their jugular veins pop at me and they have fluid in their lungs when I listen to them.
