    Sorry. My dx was impaired skin integrity related to swelling and inflammation as evidenced by break in skin and decreased mobility. I meant swelling not sweeping. So this is not acceptable? I will invest in that book, however I have to turn in the care plan in a couple of days. Thanks for letting me know about the book! :)

    I have to make a care plan for my patient who has cellulitis. She has a wound on her leg. My care plan dx is: impaired skin integrity related to sweeping and inflammation caused by a bacterial infection as evidence by break in skin tissue and decreased mobility. Does this sound right?
