Care Manager in-home education and assessment

Hi! I'm looking for a company where the nurse sees members in their homes but doesn't provide direct patient care. Not home health. More like providing education on disease processes, safety, and medications. I know of one company that does this but look for others.

try insurance companies...some of them have care manager with the role is to provide education in the home..try WellCare, Fidelis, BlueCross. Also Pharmaceutical companies also employ RNs in this role...I have seen such postings before. Non profit organizations for people with HIV or Diabetes also have jobs like the ones you are looking for.
