I know this is a weird question but I am writing an APA essay about a client. We must include two nursing diagnoses and within the paper I am talking about them such as ......"The nursing diagnosis most relevant to the client's physical state is chronic pain r/t or Chronic pain r/t or chronic pain r/t ?????

i always capitalize all the words of nursing diagnostic labels because that is the way they are presented in the official nanda taxonomy. in the taxonomy, nanda lists this as chronic pain. i would not use the abbreviation "r/t" as the related factor. the taxonomy lists related factors for each diagnosis and in their publication, nanda international nursing diagnoses: definitions and classifications 2009-2011, and describes what these are. in a formal essay i would write it out:
