Canadian Nurse to NYC forms required

Hey all,

Was wondering if anyone knew the exact forms that had to be sent as I am now a bit confused.

On this part of the NYSED department it seems like all I need to fill out is a Form1, and a Form 2.

However on this application check list.

It seems like I need a Form 1, and a Form 2F & Form 3F if I don't use the CGFNS for  my NY state (not talking about visa screen here), currently don't want to use the CGFNs given the cost at this moment.

Any help is appreciated.

Based on RN Pathway #4 form for Canadian, you needed Form 1 & Form 2 as well as Form 3F. Form 2 goes to the Canadian College and Form 3F goes to the College of Nurses for registration verification.

ADDITIONAL STEP IF YOU ARE LICENSED AS AN RN, LPN OR LVN IN ANOTHER STATE OR U.S. TERRITORY.
If you have been licensed as an RN, LPN or LVN in another state or a U.S. Territory, you must arrange to have information verifying your license(s) provided to the NYSED. 

*Form2F is only applicable if you are not educated in Canada. 

*Because CGFNS Visa Screen also requires the school paperwork including transcripts & verification of registration - they would have a copy afterwards.  After you get your VisaScreen, you can request CGFNS to send I.e. eDAS (eDocument Authentication Service - $275) to NYSED if you wanted.
