Need translation:

change LAC ML dsng q7 days

I know it's a dressing change but I'm not cluing in on LAC ML... maybe it's obvious and I'm just not getting it? It's something for my pt tomorrow.

a midline catheter is similar to a picc line, but is not threaded all the way to the svc. it often sits somewhere near the shoulder. it is good for about 2-4 weeks, shorter than the duration of a picc line but longer than a regular piv, and can be used for anything that a regular piv can be used for. sometimes when i am trying to place a picc, i am not able to get it to thread past the shoulder. in that case, i will often leave the catheter as a midline if it is appropriate for the type of therapy they are receiving (that is, no tpn). sometimes they are put in as a resus line until a someone can get in to place a large-bore central line. they don't seem to be very commen, though. i guess the thought is that if you are going to go through all that trouble, you might as well place a picc.

All sounds good to me... I have limited info b/c it's my patient for tomorrow and my instructor just gave me basic info, drugs, etc. It is hard to read my instructor's writing. I know pt has an IV with merrem, levoquin, bactrim, diflucan, & he's HIV+ but not much other info 'til morning. Whatever the dressing is, it's due to be changed tomorrow.

this is exactly what it turned out to be, they couldn't get it threaded all the way.
