California RN Endorsement and Transcript Request Forms?

I'm applying for me license to CA through endorsement. I know that we need to have our official transcripts sent to the board of nursing through the school. But my question is...do we have to have that "request for transcript" form sent to the school to send with our official transcripts?

I am able to have that request for transcript form sent out with my official transcript from the nursing school I went to.

But I went to 2 community colleges to do some general education requirements and another university for 2 years. So I can just request for my official transcripts to just be sent to the board? Or does it have to have that form with it?

I just sent my application for endorsement this August and got my license about a week ago. I was shocked how quick it was.

Yes, the form has to be sent in with it. There is a section on the form that an individual from the school actually has to fill out. I went to my university's Registrar office and we filled out the form together and I made sure my transcript looked okay and included everything before they sent it. So I know the form has to be sent in, but I'm not sure specifically if you need it sent from all of your schools or just the nursing school. If the transcript from your nursing school has all of the necessary requirements listed on it, then I assume that'd be okay:)

You only need that form sent with the transcript from your nursing school. If you need to send transcripts from other schools to verify micro or other courses, you can just have those school send the transcripts directly, no form required.

I know this is a super old post, but I just wanted to say thank you for posting this. I was so confused by the form and the transcript request process, and I can't just go to the office. I have to send transcripts from 5 different schools in 3 different states, none of which is the state I live in now. Knowing I only need the form filled out by my nursing school makes it a whole lot easier. I've been calling the BRN to ask this and can't get through. Thank you!

Hi Annaiya,

From what I've been told by other students, the blue transcript request form is to be completed by the school that issued your nursing degree. But on the safe side, I called/emailed other schools I attended for my GEs (out of town) and they gave me an address to send the form to in case they needed to sign it as well, as well as payment for the transcript.

I hope this will help other students looking for answers. If anything changes, I'll try to update my post.

Hi everyone! I'm graduating on 09/17 and have to ask 3 other schools to send in my official transcripts as well as my nursing school. Do any of you know if it's okay to send the transcripts from other schools before you graduate? I just don't want the BRN to think I have missing classes if my main nursing school's transcripts aren't sent yet. But also, I want to try to get everything sent to the BRN well in advance. Thanks in advance!
