California Online Nurse Endorsement Application Question.

I'm looking at the PowerPoint provided by California's BON that shows you what the online application is like to endorse my RN license there and came across 2 questions that I don't know how to and where to find the answer. I have provided the link to the PowerPoint below and if you go to slide 10 it asks for "RN License Number By Exam" and "RN License Issue Date by Exam" and I don't know what to put for this. I was already asked "RN License Number" and "RN License Issue Date" so I don't understand what it's asking for and where I get this information from. Can anyone please help, especially if you have done this before. Thanks !!

I looked up the link you provided in your post. I interrupted - RN number by Exam (to be the state you got licensed from completing the NCLEX) and the date you received your first license. 

Hope that helps.
