Can anyone give me more information regarding the normal standards concerning how a bronchoscope should be sterilised. Is it enough sterilising it with Gluteraldehyde in a basin or should a proper machine be used and for how long should it be left in the solution.

I'm interested in hearing how others RINSE their bronchoscopes--i.e. sterile or tap water? The standards that I've reviewed suggest sterile water if there is any chance of performing bronchoscopies on immunocompromised patients. Of course, the majority of patients receiving bronchoscopies are immunocompromised--either cancer, infection, etc.

We disinfect with CidexOPA. The Cidex insert states: "Use sterile water unless potable water is acceptable". "When practical, bronchoscopes, due to a risk of contamination from potable water supply should be rinsed with sterile water". "The use of a bacterial retentive (0.2 micron) filter system may eliminate or greatly reduce the amount of waterborne bacterial from the potable water source".

At the risk of oversimplification, a. Are not the benefits of sterilization negated if the procedure is not performed with sterile technique? b. Endotracheal suctioning is a sterile procedure when the catheter is passed below the carina, so why would a bronchoscope be used differently?
