Pretty much anywhere the guidelines state- well perfused, not a bony prominence, etc. We frequently use the back of the shoulder.
Have you been directing your questions to your preceptor? You're asking quite a few here. Your preceptor should have knowledge of the specifics of your facility's policies and procedures.

Depends on where the surgery is. Working in cardiac surgery where there is very little of the body not prepped (basically, head, arms, and back), I have always used upper back and flank.

Where do you place bovie pad for a ent surgery? Is it always thigh or are there any other sites where it can be placed?
