I need some help and recomendations for some books. I need a care plan book that will guide me in making care plans on my own for work. I work at a non profit organization and need to put together some care plans for the coaches to use for our clients. I am a new LVN and do not work under the supervision of any nurses ( I am it). My boss has authorized me $60 to spend on a good book that would help me. I also need one for a guide in documentation. I keep a nurses notes log of all incidents that happen on site and I am not sure if my wording is correct in describing things.

The book I used for Care Plans in school is

I've listed the Amazon page for this book above. It is great! (I have the earlier edition). In the front, it lists your Nursing Diagnoses; then they are each broken down with a definition, characteristics, suggested outcomes, interventions and rationales.
When we have to re-write care plans at the hospital, we use this book as well.
Thank you I did see this one on line and it did catch my eye. Im gonna check borders and see what they have in stock.
