Ive looked everywhere and cant find the answer. Does anyone know which blood product you would transfuse first, platelets or pRBC? Thank you :)
Thank you very much everyone! I appreciate the rationale behind the reasoning :)Though I believe I have the answer, the details were that shes a pediatric pt with meduloblastoma.
Wbc: 2,000/mm₃
Hgb: 7.2 g/dl
Hct: 19.3 %
Plt: 18,000
She has a single lumen infusa-port.

Ask the resident or MD if they have a preference........but I would go for the platlets because you can fly them in.......with the PLT count that low there are dangers of sopontaneous ICB's. Do they want them leuko-depleated/irradiated? or with leuko-trap type filter? with the WBC's?. Does your unit/hemeonc dpt have a specific policy reguarding theses issues? What does your charge nurse say? Is the child hemodynamically stable? I know some MD's do not tranfuse untl the HgB is below 7....but children are different.

Anything we tell you varies on MANY factors and without being able to assess the child it is IMPOSSIBLE to make a decision......and the potential of something going wrong would be catastrophic! AND not following hospital Policy, procedure and protocol can get you fire as well as leaving you vulnerable if the was a lawsuit for some reason........because you didn't follow P&P
