I'd say both. I've gotten jobs in the past both ways. Specifically, I've gotten two wonderful positions by just submitting my CV to companies that I really liked, but that didn't have job openings. Both times I received a response saying it was perfect timing because they were just about to post a job opening. Sending to places you're interested in, even if they don't have a current opening, might help them think of you when they do. When I sent my resume in blindly, I'd always say something like, though I don't see a current opening at your company/hospital etc, I have long admired the work that you do and would live to be considered for a position should you be in need of help in the future.
And of course, applying to places that actually have job openings is always a must.
I think it really depends what city/cities you are looking in. If you apply to, say a major research hospital that has no listed openings for new grads, you have a 0.00004% of getting called.
Look at where you did your clinicals, that's a great place to start. I would def recommend applying to only posted jobs.

Hi,
I just finished my degree in nursing and am excited to get started on the job hunt.
Wondering what you thought about applying to specific hospitals and units without there being a job posting versus only applying to open postings.
