As I'm sure many of you've already seen, sometimes the programs we utilize to practice NCLEX style questions are erroneous. It takes vigilant knowledge, and reviewing of the questions wrong (which one should always do in order to gain knowledge based on rationales) to find these discrepancies. I just advise my fellow nursing brethren to keep their eyes open.
I've seen issues with both the 3500:
And NCLEX 4000:
Moral of the story: Keep your eyes open, and always check your answers wrong for the rationales!
