Awesome Grand Rounds today!

We had a guest presenter at grand rounds today- a CNM who has been instrumental in the recent research surrounding delayed cord clamping. We already have a policy that all full term kids get delayed clamping (or we milk the cord if we have to hand off to NICU). However, our preemies never get delayed clamping and only get cord milking at the discretion of the provider.

Today, we talked about how to keep the cord intact for babies who need support at birth. We all know that delayed cord clamping is especially important for preemies, but only a few hospitals in the country are actually bringing resuscitation to the perineum, either with specially built resuscitation units or just making it work with standard equipment.

I don't know when we will actually start implementing this, but it was truly incredible to be in a room full of bedside nurses, midwives, obstetricians, pediatricians and neonatologists, where everyone is tossing around ideas about HOW to do this, instead of trying to come up with reasons why we shouldn't bother.

This is what happens when midwives and physicians work together!

A doctor in England has devised a bedside trolley so that you can resuscitate the neonate with the cord attached.

I looked into the BASICS device and the company is selling it for around $12,000.

The university hospital I worked at here in Denver does that. Even 23-24 weekers would get a full 60 seconds before they clamped and cut and handed off to the NICU staff. It was always a bit awesome to watch.
