I'm a student. I had a patient who was a 28 week old preemie, born almost 3 months ago. He was admitted to the nicu, mainly for respiratory problems, such as apnea.
he was discharged home with oxygen and home apnea monitor. He was later rehospitalized after 3 weeks from the discharge date related to an apneic episode. he wasnt sent to the nicu, but in one of the floors in the children's hospital. I took care of that patient for my clinical day. he weighed 5.5 pounds, they took out his NG tube coz he was tolerating his feeding well.
My question is:
1. what is the discharge plan for that patient with apnea of prematurity that is rehospitalized the 2nd time.
2. I need 1 community resource referral to a service or agency that infant and his family can use after discharge. Any suggestions. I cant find any community resource/services/agencies the family can use after discharge. Someone in the Family resource center at the children's hospital said "March of Dimes".
If you know any community resource referrals, let me know of one, who is eligible for the service, what is the referral process, and cost of service to the family.

Maybe you could call the unit and see if you could speak quickly with a social worker. NICU and PICU often have social workers on staff for just them, and they should know what kinds of resources are available in your area. :)

Where I work, the baby would get a septic workup, if on caffeine or Theo a level and a reflux workup. Some kids have to be a bit over fullterm age to kick the apnea thing. If it continues, then they probe further with a neuro workup.
In our area we have Early Intervention...free to eligible kids, all kids are referred if they are pretermers. They receive PT/OT, developmental evals. We don't really refer to March of Dimes that I know of.

Yes, bittybabygrower is right...Early Intervention is your best bet..... this baby deff is eligible for these services, being a 28 weeker. I will bet thats the one your instructor is looking for....they have social workers, pt, ot, nutrition, speech path . they are a real great resource for parents.. they stay with the kids til age 3. I work with them often, I do part time home visits for nicu graduates.
