i'm finishing up my bsn and as one of my very last classes i need to collect "interviews" on certain specialties for a paper. i did not write these questions, they are the required format that must be turned in for the class. the interviews must be turned in as well. imho these sound akin to essay questions and i apologize for them being so wordy. i am writing a summative paper based on the answers from these questions so i do not need a "paper" from each respondant. however, since i have to synthesize the answers to write my own paper i do need them to be thoughtful enough that i can write a coherent comparison/contrast for various specialty practice areas. i do not need complete sentences and anyone that has time to answer these for me would have my most sincere thanks.

I'm an adult health CNS who works as an advance practice nurse in nephrology. This is my call weekend so gotta leave in a few but let me take a stab at a few of them.

I work with dialysis pts and we care for the pt via a multidisciplinary model. I frequently consult with staff nurses, dialysis techs, social workers, and the unit manager. The techs are licensed (new requirement) personnel who do the actual tech aspects of dialysis, staff RNs oversee this care. Social workers help the pts to wind their way thru medicare rules/regs and insurance. The manager of the unit is responsible for the 24/7 operation.

I always enjoy reading your posts on the forum. I read a thread where you an llg were talking about CNS vs NP and it really influenced my decision to start the NP program this summer instead of the CNS because I couldn't find a dual program that was a good fit. At first glance I'm really more drawn to the CNS side but my ultimate career goals are going to mean I need prescriptive ability and I never know with my husband in the military which state we'll get sent to next.

My husband is retired Air Force so I'm way familiar with moving and I think you are right that if you want to have prescriptive authority you go for the NP.

1. As I coordinate care for my patients, I involve whatever discipline is needed to meet my patient's needs. The wound care nurse, social worker, ostomy nurse, pharmacist, dietician, physical therapists, doctors, speech therapist and respiratory therapist are team members I engage regularly.

2. I share the problem and either suggest how they can assist me or ask for advice on how to prevent complications.

The CNS is an expert and thus knows the best care practice and promotes evidence based practice.The CNS acts as an educator, writes policies and translates research findings to bedside practice, which is taught to the staff nurse. The nrse executive reinforces what is stated by the CNS by also writing policies and enforcing the policy.

5. Nurse-sensitive indicators are monitored thru chart review, walking rounds, secret shoppers and interviews with patients.

6. Nurses facilitate patient-entered care through nursing bedside rounds, interdisciplinary rounds, Schwartz rounds and regular family meetings.

7. I cared for a paranoid schizophrenic with lung ca during the last 4 weeks of life. We held biweekly meetings with his family and all disciplines collaborated to promote his dignityand keep him comfortable and safe.

8. Interdisciplinary bedside rounding has decreased call light use, improved collaboration and decreased length of stay.

APNs are NPs and CNS who are master's prepared nurses. A CNS is a clinical expert for a specific patient population,answers consults and is a clinical expert and in my facility, runs a clinic. The APN instructs the staff nurse and the CNL on best care protocol for a clinical problem. Depending on the state inwhich they practice, the APN has prescritive powers.

The CNL is a master's prepared nurse who focus is care coordination, patient safety and quality of care. The CNL will teach staff nurses and patients on best practice protocol for a clinical problem or seek the expert who teach the staff. The CNL collaborates with the social worker to guide the patient/family as they transition to the next level of care while coaching staff nurses to implement evidence based practice and improve quality of care. The CNL also works with the nurse manager to develop competencies for the microsystem and develop policies that promote patient safey and quality of care.

As a Hepatology ARPN (I am a Med-Surg CNS with APRN, BC) I utilize other disciplines all the time. I utilize Mental Health and Substance abuse counselors all the time, as Depression and Substance Abuse were crucial issues that cause patients to develop liver problems.

I also worked closely with their Primary Care Provicers to be sure all aspects of their health issues were being addressed, i.e., obesity, diabetes, htn, etc.

2. Describe how you demonstrate appropriate teambuilding and collaborative strategies when working with interprofessional teams. I always perceived that we are here for the patients. Therefore, it takes a "village" to take care of all aspects of complex patients. I am also a "Transplant Coordinator". The Transplant process requires many people, and disciplines, to be involved in preparing a patient and their support person in preparing for a transplant.

3. How do you see care applied within your particular nursing practice situation? As an APRN, I felt there were several important aspects of my practice. 1) diagnosis/treatment plan, mental health evaluation, educational needs evaluation, evaluation of planned/implemented interventions. When patients repeatedly said to me, "I have been to my doctors in my life, but no one has ever explained things the way you do. Now, I understand why this diet, medications, and life style changes are so important".

CNS- educator of patient/family including follow up and education for effectiveness, Nurse Executive- mangement and implementation of nursing programs with oversight for the safety of nursing programs under their direction, Bedside Nurses-implementation of acute care safety prevention, such as fall risk, medication administration, prevention of skin breakdown and nosocomial infections such as IV and urinary catheter related infections.

5. Describe how you apply quality improvement processes to implement patient safety initiatives and monitor outcomes. How are nurse-sensitive indicators monitored in your institution? Application of QI have been triggered by incidents and patient problems. Therefore, QI has been a process as my two programs have developed. Nurse sensitive indicators have evolved from interests, observations of problems, and research based questions.
