With the unit I'm on now, people tend to take report on a blank sheet of paper -- probably helps that there's been no turnover for the last 39084309843 years so they know what they're doing, lol.
With the unit that I did my capstone/practicum on, there were several standardized report sheets floating around which I found immensely useful. Unfortunately, I didn't think to save one.
Have any of you guys found a good report sheet that you've used and would be able to share?
I love this so much - I tweaked it a bit to suit my needs, but this is what I was looking for. The format makes sense to me, and all it needed was a bit of a personal touch - PERFECT thank you so much!!
