Antipsychotic medication error

300mg of clopixol depot was administered instead of 150mg. Pt has schizophrenia and receives 150mg every fortnight and was a week late in getting it.

do you think the pt will have any adverse reaction? Max dose that can be administered can be as high as 600mg weekly from what I know.
doctor seems to think it will be fine as believes she is being under medicated anyway.   ?‍♀️

I think you already have your answer.

Appears the double dose will not effect the patient negatively. The event still needs to be reported.. to prevent future errors.

Yep all done. Small medical team.
Dr checked med (obviously not properly - lapse in concentration)

so it’s both on Nurse and Doctor.
I was worried about the pt and anxiety is in over drive thinking pt could be harmed being given a double dose.

Stupid stupid mistake! ??
