I am working in a multi-specialty clinic and want to fine out if other clinics have patients sign a general consent to be examined prior to a clinic visit. along with information as to the patient rights. This would be different than an informed consent for treatment for invasive procedures or immunizations.
Eg. I voluntarily consent to recieve medical and health care services provided by XXXXXXXXXXX clinic. I understand that such services may include diagnostic procedures, examinations and treatment.
Any feedback would be creately appreciated.

I work at an Urgent Care Clinic and the patient or guardian signs a consent for treatment as well as the Patient rights form. Evcen if they are bleeding or hobbling, they have to sign the consent before we can take them back.
