I recently entered a monitoring program and thanks to this sub, I was able to go into the agreement better informed than if I never found the nurses/healthcare workers sharing their stories. Thank you all.
I was hoping to create a master thread of ALL or as many as possible ATD monitoring programs available in each state (if applicable). I did a search however my keywords did not yield a thread similar to what I was looking for. If there is one, please link it so that others and I can benefit from this information.
I am kindly asking for any input from former or current participants in any monitoring program in any state so that this thread can be updated. Please feel free to suggest any edits, additions or recommendations to keep this thread beneficial for all. I have added the general details for the current program I am in below in a rough outline.

Cost range: At least $1000 per year to BON plus Evaluations and drug test costs
License restrictions: Narcotics restriction for minimum 6 months and any restrictions given during eval

Cost range: free, except the cost of testing (so far $5 at the site and $22 charged later through Recovery Trek)
License Restrictions: minimum of no travel nursing, no home health care, only one nursing job at a time, and no nursing jobs outside the state of Colorado.
