Hi good afternoon, I need urgent assistance with my assignment (submit on tuesday) and i just dont know how to get started .

I need to make use a fictional client in aged care setting (is about aged care resident)

1. what procedures did you follow to identify and priotrise the clients needs?

2. What impact did the client's dual/ multiple diagnosis have on the prioritisation of needs?

3. How were the job roles and responsibilities of the client's carer identified and delegated?

4. which health professionals were involved and what advice or services did they provide?

5.Explain how the fact that the client has complex care issues will affect the carer and the health care plan?

6.Explain how your organization utilise best practice guideline and the standards and uidelines for aged care when developing strategies to address complex and or special needs?

7. How did you involve the client and other relevant stakeholders in the negotation and development of plans?

8. How were resources accessed and/or negotiated in order to deliver identified services?

9. What community support agencies were used to achieve the client's goal and how were they accessed?

10. If a care worker no longer able to provide the level of service required for the client how would this be handle?

11. If a carer needed respite, what respite support would be available and what processes would be followed to access it?

12.Explain the methods that would be used to collect, collate and analyse feedback from this client. From whom would feedback be gathered and why are these sources relevant?

13.If the client's goals were not being achieved, what would you do?
