Additional “Testing Fee” on drug test

Review date for release from FL IPN was last Tuesday.  I received an email saying I met  riteria first release but to continue checking in and completing all IPN requirements until I received the official exit letter.  Woke up Wed to a hair test and did that (to the tune of $300).  Today I got selected for something I’ve never seen or heard of from other participants in my five years.  It’s a test that is $99.00 with a collection fee of $10 but the. An additional $57 “test fee”.  

Has anyone seen this before?  What on earth is the additional $57 for?

I can't make heads or tails of the fees and charges:(

I am in NC and they use FS Solutions; now Vault Health, for testing, and there is no system for sending you an invoice or bill. You just get a surprise on the 13th of each month when they take their piece of the pie.

When I asked them why, they directed on how to navigate their site to see my billing history, and said that I can tell from looking there... really? I took a screenshot of my billing history.

Maybe I'm stupid, but I can't look at this and tell how much my next bill will be?
