I am doing a care plan and my diagnosis is Activity Intolerance and I am having a hard time coming up with a goal? The client is unable to care for himself, his caregiver is doing everything for him pretty much. He is HIV positive and is recovering from pnewmoscystitis carinni pneumonia. He is weak and tired, short of breath, hot and cold. He is wetting the bed. Any help with coming up with a related to and a goal would be much appreicated!

What about VS will remain stable after dangling, turning, ambulating?
O2 sats will not drop below 90% during length of shift?
here is what I came up with, and it doesnt have to be right, we just have to try so please dont make too much fun of me if I am way off! lol.
I put Client will show signs of activity tolerence 1 time without showing signs and symptoms of physcial distress during my shift of so and so.
I'd specify what kinds of signs, otherwise what do you evaluate and assess specifically, you know?

so were we all at one time. this stuff doesn't come to you overnight. it'd be great if it did! i didn't really get the hang of care plan writing until i was out of nursing school and in a job where we were required to do it. then i learned up pretty darn fast and realized how much i hadn't been paying attention in nursing school.
