About CES Application

I have a question related to Credential Evaluation Services...I don't know how to fill out some part of it specially big confusion about where to send the Registration/Licence and also Acedemic Records (Transcript) form...I have done RN from India and I need to done my CES report soo please can anyone tell me where to send these forms it'll be big help for me.....Thankz

    You need to arrange with your licensing body and university (or place where you trained) to send verification of license and transcripts to CGFNS

    You need to arrange with your university to send your transcripts to CGFNS. If you send them they must be in a sealed envelope stamped along the seal with the university stamp to ensure that no tampering is done.

    You need to arrange with your nursing licensing body to send verification that you are a RN to CGFNS.

    Generally documents like this must come directly from the establishment therefore you need to complete the forms from CGFNS and send to the department responsible to send your documents and they must include these forms with their documents so CGFNS know which file to put them in
