Abdominal Vaginal Preps?

Hello,

I have a question for you? How are you all doing abdominal/lady partsl preps for your TLH's? Do you have a protocol or policy regarding lady partsl prep solutions? Chlorhexidine vs Betadine???

I appriciate any help with this. I need evidence based criteria for a policy.

We use chlorhexidine soap (Endure) in the lady parts if the patient is allergic to betadine.

AORN magazine this month discusses this. lady partsl prep first to prevent splashing, then abdomen. But the key to remember is to think of this as two separate sites. I always think of the traditional supine position for an abdominal hysterectomy-you frogleg the patient, put in the foley and do a vag prep. The same would go for a stirrup case. Start at the bottom. Stop change gloves and do the abdomen with a clean set up. It is not recommended to use CHG in the lady partsl area, or on mucous membranes. It just causes too much irritation in that area. So I would stick to manufacturer's guidelines. I have started to see betadine preps in the perineal area with a one step prep, like duraprep or chloraprep for the abdomen, which is good and saves time, since most of the doctor's now are not doing the traditional hand scrub at the sinks. It's very stressful to do these types of preps while people are rushing you and thinking you are taking too much time, even though you are doing the prep properly. As far as policy, It should just follow your hospital approved solutions, and the hospital recognized nursing procedural texts.

Try the recommended practices and guidelines book or get online access. I think that would be more help than trying to track down a journal article, and they usually have a long list of references.
